List of All Prescribed medications - please include the name of the medication, the dosage and how it is prescribed. Please include the name of the prescribing physician.
For the Chronic Pain treatment patient please, to the best of your ability, provide us with a history of medical conditions, medications and procedures that have been attempted in your treatment. Medical Records may be faxed to 949-499-7651. This is a Confidential Fax line in the Intake Office.
What to Bring:
All current medications, including all addictive substances
4-5 changes of comfortable clothing (washer and dryer on premises) Please note that there is a dress code that discourages clothing with drug related messages, as well as tight, revealing or otherwise suggestive clothing.
(All belongings will be inventoried upon admission.)
Comfortable attire for Yoga, stretching exercises and walks
Pajamas, robe and slippers
Lightweight jacket
Comfortable shoes
Hats/caps/sunglasses for outdoor activities only
Toiletries
Cigarettes (no cloves, cigars, chewing tobacco) We support nicotine cessation and will provide nicotine gum on request.
Telephone calling card for long distance calls
Eyeglasses/ contact lenses
Electrical items (i.e. hair dryer) will be approved for safety by the Engineering Department
No more than $25.00 in cash (large amounts of cash, jewelry, credit cards will be secured in the hospital safe)
Special articles from home, including small family pictures, comforters, pillows, etc.
Please leave the following items at home:
Electronic media and communication devices - including cell phones, laptops, iPods, cameras, Televisions, radios, CD players, iPods, musical instruments, and blackberry's.
Books and magazines that are not recovery related
Work/Job materials
Food, candy, beverages
Clothing that depicts alcohol, drugs, violence or sex.
Expensive jewelry
Toiletries and products that contain alcohol (i.e. mouthwash and hairspray)
Vitamins, herbs and nutritional supplements, illegal substances
